In his own words: "Upon discharge from Montgomery General Hospital there was an IV left inside my right arm. My physician's name was Myra Priddy, M.D., and she was in charge of my well being.

"A nurse... had to remove the IV. The nurse’s name (not mentioned) that removed the IV works here at Mount Olive Complex and is a witness to this incident."

Chafin asks for a jury to provide the following $10,000 for negligence, $10,000 for malpractice, a reprimand for persons involved and punitive damages.

He submitted an Affidavit Of Indigency And Application to be eligible for waiver of fees and costs involved with his case, which is docketed for June before Judge Jay M. Hoke.
