* Kathleen Holcomb was a resident of the defendants from Jan. 18, 2011, until her death on June 25, 2011. Cheryl Holcomb claims the defendants' wrongful actions caused Kathleen Holcomb's health and physical condition to deteriorate beyond that caused by the normal aging process and resulted in physical and emotional trauma, including infections, sepsis, pressure sores, dehydration and ultimately, her death. Cheryl Holcomb is seeking compensatory and punitive damages.

Holdsclaw claims in June 2009, the defendants placed T.B., a known physical and sexual predator, in the same foster home with the children and from June until August 2009, T.B. proceeded to physically and sexually abuse them until the Beckley police were alerted on Aug. 24, 2009. Holdsclaw is seeking compensatory and punitive damages with pre- and post-judgment interest.

Yellowbook is seeking judgment in the amount of $20,771 with interest at a rate of 7 percent per annum.

* On Aug. 17, 2011, Smith read in an article published in the Charleston Daily Mail of a study WVSU commissioned to help determine ways it could raise nearly $26 million in private funds. On Sept. 20, 2011, Oct. 15, 2011, and Nov. 30, 2011, Smith sent a total of three FOIA requests to Carter and in December, he received a packet with an executive summary, but it failed to satisfy the multiple FOIA requests. Follow-up letters were also not answered, according to the suit. Smith is seeking for the defendant's refusal to disclose the records as unlawful, grant injunctive relief and grant a permanent injunction.

Zona Hannah and Susan Hannah vs. Montgomery General Hospital, Inc. and Dilipkumar Parsotambhai Ghodasara, M.D.

* On Sept. 27, 2011, Zona Hannah presented to the emergency department of Montgomery General Hospital with a history of hypertension and extreme confusion, where he was seen by Ghodasara and then released to go home. Later that evening, Zona Hannah woke up, fell to the floor, was unable to get up or walk and did not know who his family was or where he was. Susan Hannah took Zona Hannah back to the emergency department, but they refused to see him because he had already been there and also refused to call an ambulance to take him somewhere else. Susan Hannah claims she and her son took Zona Hannah back into the car, drove around the corner and called 911 for an ambulance, where Zona Hannah was then taken to Charleston Area Medical Center and diagnosed as having multiple strokes and grand mall seizures. The Hannahs are seeking compensatory and punitive damages with pre- and post-judgment interest.
